If you’ve been declined for life insurance because of your health, final expense insurance is probably still available to you – and often at a better rate than you’d expect. Final expense carriers built their business around older applicants with health histories. They don’t expect perfect health; they expect diabetes, blood pressure medication, a stent from a few years ago.

The question isn’t whether you can get coverage. It’s which tier you qualify for: simplified issue (full benefit from day one, lowest cost), a graded plan (partial benefit in the first two years), or guaranteed acceptance (no health questions, two-year graded period, highest cost). Each carrier draws those lines differently, which is why TermHero compares a dozen-plus of them for every client.

Below is a condition-by-condition guide to what typically happens. It’s a starting point – call us for the specific carriers that fit you.

Common Conditions: What to Expect

Diabetes

  • Type 2, controlled with oral medication or diet – Simplified issue, full coverage from day one with most carriers
  • Type 2 on insulin – Simplified issue with many carriers, especially if diagnosed after 50 and without complications
  • Type 1, or diabetes with complications (neuropathy, kidney disease, amputation) – Graded plan or guaranteed acceptance
  • Insulin shock or diabetic coma in the last 2 years – Guaranteed acceptance

Heart Disease

  • High blood pressure, high cholesterol – Simplified issue with essentially every carrier
  • Heart attack, stent, bypass, or angioplasty more than 2 years ago – Simplified issue with many carriers; 12-24 months with some
  • Within the last 12 months – Guaranteed acceptance
  • Congestive heart failure, defibrillator, or pacemaker for heart failure – Guaranteed acceptance (a pacemaker alone for rhythm is often fine)

Stroke or TIA

  • More than 2 years ago, fully recovered – Simplified issue with several carriers
  • Within 12-24 months – Graded plan or guaranteed acceptance

COPD, Emphysema, Chronic Bronchitis

  • Mild to moderate, no oxygen – Simplified issue or graded, depending on carrier and whether you smoke
  • Oxygen use – Guaranteed acceptance

Cancer

  • In remission 2+ years (most types) – Simplified issue with many carriers; some require 3-5 years for certain cancers
  • Basal or squamous cell skin cancer – Usually ignored entirely
  • Currently in treatment, or diagnosed within 2 years – Guaranteed acceptance
  • Metastatic or recurrent cancer – Guaranteed acceptance

Kidney Disease

  • Chronic kidney disease, stage 1-3 – Simplified issue or graded with select carriers
  • Dialysis – Guaranteed acceptance

Alzheimer’s, Dementia, Parkinson’s

  • Alzheimer’s or dementia, any stage – Guaranteed acceptance (the insured must be able to consent, or a power of attorney must sign)
  • Parkinson’s – Graded or guaranteed acceptance, depending on severity and carrier

Mental Health

  • Depression, anxiety, bipolar disorder, managed with medication – Simplified issue with most carriers
  • Schizophrenia – Graded or guaranteed acceptance

Other Conditions

  • Arthritis, thyroid, asthma, sleep apnea, GERD – Simplified issue; rarely an issue
  • Hepatitis C, treated and cleared – Simplified issue with many carriers
  • HIV – Guaranteed acceptance; a few carriers now offer simplified issue if well-controlled
  • Alcohol or drug treatment within 2-5 years – Graded or guaranteed acceptance
  • Currently in a nursing home or hospice – Guaranteed acceptance with select carriers
  • Obesity – Height/weight tables vary a lot by carrier; being declined by one doesn’t mean declined by all

The Three Tiers Explained

Simplified issue (Level benefit) – You pass the health questions. Full death benefit from day one. Lowest premium. Details.

Graded / Modified benefit – Offered by some carriers to applicants with moderate conditions. Typically pays 30-40% of the face amount in year one, 70-75% in year two, and 100% from year three. Premium is between the other two tiers.

Guaranteed acceptance – No health questions. Accidental death paid in full immediately; natural-cause death in the first two years returns premiums plus interest. Highest premium. Details.

The most expensive mistake we see is someone with controlled diabetes buying a guaranteed acceptance policy from a TV ad when they’d have qualified for simplified issue – paying 30-50% more and taking on a waiting period they didn’t need.

How We Find the Right Carrier for Your Health

  1. Tell us your conditions and medications – Honestly and completely. Carriers check prescription databases, and accurate answers get you the best tier.
  2. We match you to carriers – Each company has conditions it’s lenient on. We know which ones.
  3. We apply to the best fit – Usually one application, by phone, with a decision in days.
  4. If declined, we already have plan B – Another carrier or a guaranteed acceptance policy, with no gap.

Frequently Asked Questions

Will the carrier find out about a condition I don’t mention? Likely yes – final expense carriers run electronic prescription history checks. Misrepresenting your health can void the policy during the two-year contestability period. Tell us everything; there’s almost always a carrier that will accept it.

I was declined by one company. Am I uninsurable? Almost never. Carriers’ guidelines differ widely. A decline from one is not a decline from all – and guaranteed acceptance has no health questions at all.

Does taking a lot of medications hurt me? Only if the medications signal an excluded condition. A 72-year-old on five common medications for blood pressure, cholesterol, thyroid, and arthritis is a routine simplified issue applicant.

Does my weight matter? Some carriers have height/weight limits; others don’t. If you’ve been told your weight is a problem, we’ll find a carrier where it isn’t.

Can I upgrade later if my health improves? Yes. If you buy guaranteed acceptance now and your condition stabilizes for two years, you can apply for simplified issue and, if approved, replace the more expensive policy.
